We offer flexible membership plans for individuals and families to make quality primary care accessible. This is not an insurance plan.

Membership Pricing:

• Individual: $65/month
   
• Family: $150/month
   

Enrollment Details:

• A contract must be signed upon enrollment.
   
• Individuals: First and last month collected upfront ($130 total).
   
• Families: First and last month collected upfront ($300 total).
   
• Monthly fee will then be automatically billed on the 5th of every month.
   

Membership Benefits:

• Comprehensive wellness exam
   
• Client lab pricing
   
• Two visits per month (additional visits $15 each)
   
• Free medication refills
   
• Access to your provider via text
   

Contract Terms:

• Contracts are valid for 6 months and can be renewed.
